There are
two additional
regulatory requirements for shipments made from outside the United
States
(note that Mexico and Canada are NOT exempt from these requirements).
- First,
a toxic substances control act form must be completed
(TSCA form).
Ask us and we will fax
you the form. The form should be given to the company
which is shipping
the dewar (e.g. FedEx, UPS, TNT, or DHL), and a copy should be placed
inside
the dewar. The TSCA form is completed jointly by the sending and
receiving
parties. In practice, this means that we will fax you a form which you
should
complete by entering your name, address, and shipping company
information.
- Second,
you should hand out on a paper the shipping company answers to the
following questions (include a copy in the dewar also):
Name of
the commodity (i.e. name of the protein):
1. Is the commodity produced
in non-infectious bacteria? (YES)
2. Does the commodity
contain animal additives? (NO)
3. Is the commodity used for
research only? (YES)
4. Is the commodity
infectious? (NO)
5. Is the commodity
hazardous? (NO)
6. Is the commodity
cultured? (YES or NO)
7.
Is the commodity recombinant? (YES or NO)
8.
Does the commodity contain bovine serum albumin? (YES or NO)
Only
the last three questions normally have different possible answers given
the
type of work we do (the answer to the first five questions is almost
always as
shown).
